Before leaving the hospital

Understand the diagnosis and recovery plan, get written discharge instructions, confirm all medications (and what changed), schedule follow-up appointments, and ask about warning signs to watch for and who to call.

Setting up the home

Stock easy meals and essentials, clear safe walking paths, set up a rest area with things within reach, and arrange any needed equipment.
