How we score it (in plain sight)
Transparency matters to us, so here's exactly how it works: each answer is worth 0–2 points based on how much it may affect safety or wellbeing, and a few answers (recent falls, frequent worry, or a hard hospital recovery) are weighted as priority flags. The tool adds these up to suggest one of four honest reads — thriving at home, a few areas to watch, time to consider more support, or let's talk soon — and highlights the specific areas where help may matter most. It's a conversation starter, not a diagnosis.
