Before leaving the hospital
- We understand the diagnosis and the recovery plan
- Written discharge instructions in hand
- Full, updated medication list — and what changed
- Prescriptions filled or a plan to fill them today
- Follow-up appointments scheduled (with dates/times)
- We know the warning signs to watch for — and who to call
- Any equipment (walker, commode, oxygen) arranged
- Home health or therapy referrals set up, if ordered
- Transportation home is arranged
Setting up the home
- Easy meals and essentials stocked
- Safe, clear walking paths; fall hazards removed
- Rest area set up with items within reach
- Medications organized with a reminder system
- A fast way to call for help in place
The first days home
- Medications taken correctly and on time
- Watching for warning signs; symptoms noted
- Follow-up appointments attended (transportation arranged)
- Rest and gentle movement as advised
- Eating and drinking well
- Care team updated on any concerns
